ADA Paratransit Recertification Application

As per Easy Lift Transportation policy, it is required that all Dial-A-Ride passengers are recertified for eligibility every three years.This serves two purposes:

  1. To ensure all Easy Lift pasaseners continue to qualify for our services
  2. To help maintain an up-to-date database to serve you better

Please only complete the Application for Recertification if your Dial-A-Ride eligibility has recently lapsed. If you are unsure or have specific questions about the recertification process, please contact the Eligibility Coordinator at (805) 845-8963.
